Summit Lean
Telehealth Consent
Last updated October 16, 2026
Telehealth means getting medical care from a licensed Provider using electronic communication, such as secure messaging, online forms, or video, instead of an in person visit.
By continuing, you understand and agree that:
- Your care is provided by independent licensed Providers, not by Summit Lean.
- Your Provider may decide that telehealth is not right for you and that you need an in person exam.
- Telehealth has limits. Your Provider cannot physically examine you and relies on the information you give, so it must be complete and accurate.
- Electronic communication carries some risk, including technical failures and, rarely, breaches of privacy despite reasonable safeguards.
- A prescription is issued only if your Provider determines it is clinically appropriate.
- Compounded medications are not FDA approved. Your Provider will explain risks, side effects, and alternatives, including FDA approved medications.
- You can withdraw this consent at any time by emailing [email protected], without affecting your right to future care.
- Telehealth is not for emergencies. In an emergency, call 911.
You also agree to tell your Provider and your regular doctor about any medication you are taking and any change in your health.
