This policy explains how Summit Lean LLC ("we") collects, uses, and protects your information.
Contact details (name, email, phone, state), quiz answers, payment information (processed by our payment processor, not stored by us), and site usage data such as device, browser, and pages visited.
Medical intake and health information you give to Providers is collected and stored by our clinical partner's HIPAA compliant platform and is protected health information under HIPAA. Summit Lean only accesses it as needed to support your care and under a business associate agreement.
To check eligibility, connect you with a Provider, process payments, send order and shipping updates, provide customer support, improve our services, and, with your consent, send marketing messages.
If you opt in, we send text messages about your account and, separately if you consent, promotional messages. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. We do not sell or share your phone number or text message opt in data with third parties for their marketing.
We share information only with Providers and our clinical partner, pharmacies that fill prescriptions, payment processors, shipping carriers, and service providers who help run our business under confidentiality obligations, or when required by law. We do not sell your personal information.
We use cookies and similar tools to run the site and measure advertising. We do not send health information to advertising platforms.
You can unsubscribe from emails using the link in any email, opt out of texts by replying STOP, and request access to or deletion of your information by emailing [email protected]. Some records must be kept by law.
We use reasonable safeguards to protect your information. No system is completely secure.
